Hospice Criteria
- The patient has a chronic or terminal illness, such as: Alzheimer’s, ALS, cancer, cardiac disease, debility, dementia, HIV/AIDS, liver disease, pulmonary disease, renal disease, stroke, and others.
- A terminal diagnosis with a life expectancy of six months or less, if the illness runs its natural course.
- The patient’s primary physician and hospice medical director approve hospice care.
- The focus of care is palliative, emphasizing pain management, comfort, and emotional or spiritual support.
